Process Forms


The Discipline Complaint Form is used to file a complaint against a student, fill out the Discipline Complaint Form below and return it to 024 Classroom Building. Provide as much detail as possible in your complaint, giving dates and times of events and names and contact information for all witnesses:

The Witness List is to be completed If there are multiple witnesses to an incident, to provide names and contact information for all individuals who have relevant information about a complaint that has been submitted.

The Witness Statement is to be completed by people who observed a violation of the Code of Conduct.  Witness statements can be submitted in support of a complainant's position or a respondent's position.

The Appeal Form is used to appealing a decision that was reached at a Level II or Level III Hearing or certain Greek Life Judicial Boards.
